Job Description

The Clinical Care Navigator (PRN) at NeuroFlow will work directly with patients to connect them to mental healthcare providers, respond to referrals, and provide crisis intervention services. They will manage a caseload of clinically complex patients, conduct risk assessments, and coordinate with internal stakeholders to ensure patient safety.

Responsibilities

• Manage a clinical caseload of patients
• Triage potential crisis calls and conduct risk assessments
• Provide crisis intervention services and safety planning
• Provide continuous evaluation and follow up for patients
• Coordinate with internal stakeholders to ensure patient safety
• Support non-clinical teammates in managing complex cases
• Utilize NeuroFlow’s technology, tools, messaging, and workflows
• Prioritize daily workloads to ensure timely care

Requirements

• 2+ years' clinical/crisis intervention experience
• Master's in a clinical field with state licensure OR RN with psych experience
• Experience and comfort with fast paced environment
• Demonstrated clinical skills in risk assessment, safety planning and crisis intervention
• Professional and organized communication skills
• Ability to build rapport and provide support virtually
• Passion for using technology to improve access to behavioral health services
• Strong insight and judgment, and the ability to make quick and clinically accurate decisions
