Job Description

Communities Care Coordinators provide personalized service to connect members with high-quality providers, answer healthcare benefit questions, initiate healthcare services, and assist with navigating the healthcare system.

Responsibilities

  • Answering inbound calls and chats to provide support
  • Owning the member's healthcare journey
  • Collecting and documenting member information
  • Contributing to the provider directory
  • Supporting clinical services
  • Following set processes
  • Advocating and navigating members
  • Educating members on benefits
  • Triaging inquiries
  • Connecting members to clinical services
  • Upholding the company's mission
  • Driving strong outcomes
  • Collaborating with other departments

Requirements

  • Exceptional business writing skills in English and Spanish
  • Proficiency with Google Apps, Apple Products, and CRM software is preferred
  • Familiarity with VoIP phone systems
  • Bachelor’s degree or 2+ years of relevant work experience is preferred
  • Excellent customer service skills
  • Exceptional critical thinking and problem-solving skills
  • Successful handling of confidentiality, security, and HIPAA guidelines
