Job Description

Join Aledade as a Practice Transformation Specialist (PTS) and make an impact on patient care. PTS are HQ's direct interface with providers, gleaning insights into partner practice happiness and needs. A successful PTS is passionate about population health and mission-oriented around providing better care while reducing costs. The role impacts community health by transforming one practice at a time.

Responsibilities

  • Implement products and execute ACO initiatives with Market Leadership direction
  • Implement and support wellness visits, transitions of care, active management, ensuring practices meet goals in Practice Transformation Plans
  • Assist in training office staff, physicians, and providers on processes and technologies (coordinating with Aledade's Value Based Care Curriculum)
  • Continuously assess practice and staff training needs
  • Develop and nurture lasting relationships with physicians, providers, and healthcare system executives to support financial and quality performance
  • Listen to practice concerns, relaying important information to Manager/Leads to understand opportunities for improvement

Requirements

  • Resident of Northern California
  • Graduate of an accredited college or university, or 4-6 years of significant and relevant medical practice management experience in lieu of educational requirements (particularly with significant administrative experience in a clinic setting)
  • 3 years of experience with Electronic Health Records (EHR) for clinical/practice management processes
  • Excellent oral and written communication (public presentation skills), organizational and project management, and team-building skills
  • Familiarity with process mapping and workflow analysis tools
  • Excellent computer skills and willingness to learn additional software applications
  • Ability to sit for prolonged periods, extensive computer and keyboard use, occasional walking and lifting, ability to travel up to 30% in Northern California
