Form Health is a virtual obesity medicine clinic seeking physicians to provide telehealth obesity medicine and medical weight management care. The role involves collaborating with a multidisciplinary team and requires a commitment to high-quality, evidence-based care.

Responsibilities

• Provide longitudinal telehealth obesity medicine and medical weight management care
• Collaborate with advanced practice professionals and registered dietitians
• Commit to a full-time schedule seeing patients across a 5-day work week

Requirements

• MD or DO degree
• Board Certified in Obesity Medicine (ABOM)
• 2+ years of obesity medicine/medical weight management experience
• Licensed to practice and prescribe in specified states
• Excellent interpersonal skills
• Commitment to high-quality, evidence-based obesity management
• Prior experience conducting telemedicine visits
• Prior experience working collaboratively in a multi-disciplinary fashion
• Bilingual candidates with medical fluency in Spanish are highly preferred
