Job Description

Pair Team is seeking a highly motivated full-time Bilingual Community Engagement Specialist to engage with and enroll individuals living with Serious Mental Illness/ Substance Use Disorder, experiencing homelessness, and/or those who have high medical needs. The role involves virtual and in-person outreach, education, and enrollment, with a focus on building trust and relationships to improve access to healthcare.

Responsibilities

• Identify, outreach, engage and enroll individuals into the program virtually and in-person
• Listen openly to eligible members and meet them where they are
• Educate partners regarding Pair Team’s services
• Explain the services and benefits of Enhanced Care Management
• Leverage community understanding and customer service skills to gain trust and enroll members
• Promote effective communication amongst internal teams and partners
• Meet key enrollment metrics
• Manage the end-to-end enrollment funnel
• Provide consistent updates on funnel stages
• Proficiency in all additional internal/external technology tools utilized for the position

Requirements

• Physically located in California
• Reliable transportation for engagement at clinic, community based organization, and health system partner locations
• Virtual Ops requires a quiet, HIPAA compliant and internet connected space
• 1+ year of experience working in a metric or quota-driven environment
• Dedicated to working within and improving the community
• Results driven with a proven track record of exceeding performance indicators
• Bilingual (English/Spanish) with excellent communication skills
• Strong technical skills and comfort with utilizing multiple technology platforms
• A valid driver’s license and auto liability insurance
• Fantastic listener, skilled at “reading people”
• Experience with high volume outreach and engagement campaigns
• Reliable and comfortable in an environment that is ever-changing
