Job Description

Pair Team is seeking a full-time Bilingual Family Nurse Practitioner to provide virtual preventive care for underserved patients, develop clinical programs, collaborate with the product team, and work with a diverse team. This role involves engaging with individuals facing complex healthcare and social needs, such as serious mental illness, substance use disorder, or homelessness.

Responsibilities

• Provide virtual preventive care
• Develop and refine clinical programs
• Work with the product team to improve the platform
• Collaborate with a diverse team
• Engage with patients with complex social and healthcare needs

Requirements

• 2+ years of clinical experience serving patients with complex needs
• 1+ year of clinical case management experience preferred
• Experience working with a multidisciplinary team
• Experience with digital-first healthcare and various tech platforms
• Board certification or eligibility in Family Medicine
• Comfortable working with patients across the lifespan
• Experience working with Medicaid/FQHC clinics
• Bilingual (Spanish and English)
• NP license in CA
• Active DEA license preferred
