Job Description

Pair Team is looking for a full-time Behavioral Health Care Manager to provide mental health resources and therapeutic interventions for individuals with Serious Mental Illness, Substance Use Disorder, homelessness, or high medical needs. The role involves working in a team-based model, providing short-term interventions, and linking individuals to community supports. The position is primarily remote with 1-2 in-person visits per month.

Responsibilities

• Work with individuals struggling with severe mental illness or substance use disorders
• Provide short-term interventions and behavioral activation strategies
• Provide intermediary interventions and referrals to community supports
• Provide brief crisis intervention and warm hand-offs to crisis resources
• Listen to individuals and meet them where they are
• Assist with linkage to ongoing behavioral health treatment
• Support ECM team's caseload to identify and address barriers to care
• Educate individuals on behavioral health conditions
• Work with individuals to identify health goals and incorporate them into a care plan
• Collaborate on care issues with the ECM team
• Facilitate and ensure recommendations are communicated across the healthcare team
• Track and manage caseloads using electronic medical records
• Consult with the ECM team on clinical concerns
• Provide educational training on behavioral health conditions and treatment

Requirements

• LCSW, LMFT, or LPCC license issued by the state of California
• Bilingual English/Spanish
• Strong understanding of cultural fluency
• Demonstrated experience working with individuals experiencing complex chronic needs, homelessness, or SMI/SUD
• Strong technical skills and comfort with technology
• Physically located in or near California
• Reliable transportation (if Field Ops) or HIPAA-compliant workspace (if Virtual Ops)
