Job Description

Pair Team is seeking a full-time Bilingual Licensed Clinical Social Worker to provide mental health resources and therapeutic interventions for individuals with Serious Mental Illness, Substance Use Disorder, homelessness, or high medical needs. The role involves a team-based approach with other care professionals and combines remote work with occasional on-site visits.

Responsibilities

• Work with and support individuals struggling with severe mental illness or substance use disorders
• Provide short-term interventions and behavioral activation strategies
• Provide intermediary interventions as a bridge to community supports and referrals
• Provide brief crisis intervention and warm hand-offs to crisis resources
• Listen openly to individuals and meet them where they are
• Assist with linkage to ongoing behavioral health treatment
• Support ECM team's caseload to identify and address barriers to care
• Educate individuals on behavioral health conditions
• Work with individuals to identify health/wellness goals
• Collaborate on care issues with the Enhanced Care Management team
• Facilitate and ensure recommendations are communicated across the healthcare team
• Track and manage caseloads using electronic medical records
• Consult with ECM team about clinical concerns, provide educational training

Requirements

• LCSW, LMFT, or LPCC license issued by the state of California
• Bilingual English/Spanish
• Strong understanding of cultural fluency
• Demonstrated experience working with individuals experiencing complex chronic needs, homelessness, or SMI/SUD
• Strong technical skills and comfort with technology
• Physical location in or near San Diego, California
• Reliable transportation (if Field Ops) or HIPAA-compliant workspace (if Virtual Ops)
