Job Description

Maven Clinic is seeking a mission-driven health coach to join their Care Delivery team. As a Care Coach, you will coordinate care for members, drive behavior change through goal setting, and improve outcomes for higher-risk members. This role involves conducting appointments, providing health education, collaborating with providers, and monitoring member health data.

Responsibilities

• Convey the benefits of the Maven platform and engage in care coaching to optimize program opt-in rates
• Utilize coaching framework and principles to drive the creation of SMART goals with members
• Conduct introductory and follow-up appointments, establishing a longitudinal relationship
• Adhere to clinical program guidelines and protocols, utilizing motivational interviewing skills
• Assess member knowledge and barriers, providing personalized health education
• Collaborate with providers to update and manage member care plans
• Remotely monitor member health data and activities, following protocols and procedures
• Ensure seamless access to care through coordination within the Maven network and external parties
• Participate in efforts to improve quality of care and clinical outcomes
• Provide coverage to ensure all members receive appropriate care
• Collaborate with cross-functional stakeholders
• Anticipate and address member’s social determinants of health needs
• Any other duties as assigned

Requirements

• 1-2+ years experience in health coaching
• Must be permanently located in one of the specified states
• Ability to remain flexible and have a growth mindset
• Ability to communicate empathetically and passionate about creating a positive member experience
• Experience achieving KPIs relevant to patient outcomes
• Health coaching certification (NBC-HWC or ICF), or personal training certification
• Proficiency in navigating new technologies and systems
