Job Description

As a Community Health Worker, you will collaborate with physician organizations, medical providers, and social services agencies to provide short-term care coordination related to Social Determinants of Health (SDOH). This includes linking patients to community resources, assisting with health improvement, and providing education.

Responsibilities

• Conduct outreach to identify community resource needs
• Conduct SDOH screenings
• Assist patients in developing self-management plans
• Document patient encounters and maintain electronic files
• Facilitate communication between providers and patients
• Provide appropriate billing codes
• Maintain a community resource database
• Participate in care team meetings

Requirements

• High school diploma or GED
• CHW certification or willingness to obtain
• 1 year of relevant experience
• Resident of Michigan or bordering state
• Strong interpersonal and community skills
• Knowledge of community resources
• Working knowledge of outreach programs
• Ability to plan and evaluate patient care plans
• Computer skills (email, excel)
