Job Description

The Health Navigator will collaborate with the interdisciplinary care team to improve health plan member health outcomes and support population health initiatives through telephonic outreach, social determinants of health screenings, and care gap closure.

Responsibilities

• Conduct telephonic outreach to health plan members
• Complete social determinants of health screenings
• Facilitate access to community resources
• Engage members in preventative screenings
• Coordinate and document member adherence
• Assist members in understanding care plans
• Build relationships with members, providers, and care teams

Requirements

• Familiarity with the healthcare community
• Valid Delaware driver’s license
• 1-3 years of healthcare experience
• Ability to collaborate with care teams
• Experience with electronic health records
• Strong organizational and communication skills
• Access to reliable high-speed internet
• Associates Degree
