Aledade is seeking a Practice Transformation Specialist to support New Jersey practices and impact patient care. The role involves implementing products, providing training, and building relationships with providers to improve financial and quality performance. The ideal candidate is passionate about population health and reducing healthcare costs.

Responsibilities

• Implement products and ACO initiatives
• Support wellness visits, transitions of care, and active management
• Train office staff and providers on processes and technologies
• Assess practice and staff training needs
• Develop relationships with physicians and healthcare executives
• Relay practice concerns to Manager/Leads
• Ensure practices meet goals in Practice Transformation Plans

Requirements

• Must be a resident of New Jersey
• Bachelor's degree or 4-6 years of medical practice management experience
• 3 years of experience with Electronic Health Records (EHR)
• Excellent communication, organizational, and project management skills
• Familiarity with process mapping and workflow analysis
• Strong computer skills
