Aledade is seeking a Practice Transformation Specialist to support Ohio practices and impact patient care. The role involves implementing products, providing training, and building relationships with providers to enhance performance and achieve goals in value-based care.

Responsibilities

• Implement products and ACO initiatives
• Support wellness visits, transitions of care, and active management
• Train staff and providers on processes and technologies
• Assess practice and staff training needs
• Develop relationships with physicians and healthcare executives
• Relay practice concerns to Manager/Leads
• Travel to practices up to 20% within Ohio

Requirements

• Must be a resident of Ohio
• Bachelor's degree or 4-6 years of medical practice management experience
• 3 years of experience with Electronic Health Records (EHR)
• Excellent communication, organizational, and project management skills
