Aledade is seeking a Practice Transformation Specialist to support South Carolina practices and impact patient care. The role involves implementing products, providing training, and fostering relationships with providers to improve financial and quality performance. This position requires a mission-oriented individual passionate about population health and reducing healthcare costs.

Responsibilities

• Implement products and execute ACO initiatives
• Support wellness visits, transitions of care, and active management
• Train office staff and providers on processes and technologies
• Assess practice and staff training needs
• Develop relationships with physicians and healthcare executives
• Relay practice concerns to Manager/Leads
• Travel to practices up to 25% in South Carolina

Requirements

• Must be a resident of South Carolina
• Bachelor's degree or 4-6 years of medical practice management experience
• 3 years of experience with Electronic Health Records (EHR)
• Excellent communication, organizational, and project management skills
• Familiarity with process mapping and workflow analysis
• Excellent computer skills
