Job Description

Aledade is seeking a Practice Transformation Specialist to support Tennessee practices and impact patient care. The role involves implementing products, providing ACO training, and ensuring practices meet performance goals. This position requires a mission-oriented individual passionate about population health and reducing costs.

Responsibilities

• Implement products and execute ACO initiatives
• Support wellness visits, transitions of care, and active management
• Train office staff and providers on processes and technologies
• Assess practice and staff training needs
• Develop relationships with physicians and healthcare executives
• Relay practice concerns to Manager/Leads
• Travel to practices up to 20%

Requirements

• Must be a resident of Tennessee
• Bachelor's degree or 4-6 years of medical practice management experience
• 3 years of experience with Electronic Health Records (EHR)
• Excellent communication, organizational, and project management skills
• Familiarity with process mapping and workflow analysis
• Excellent computer skills
